Description
The countywide Diabetes Multidisciplinary Team (DMT) promotes the self-management of diabetes to avoid acute and long-term complications. It provides one-to-one clinic appointments and group education courses, which are taught by specialist diabetes educators. The team also offer telephone and email support for people with diabetes and their families.
The service offers locality-based clinics for patients who need to be accommodated urgently, and for those with type 1 diabetes with highly complex needs. The clinics also offer services outside of working hours. The teams consist of specialist consultants, dietitians, nurses, support workers, and diabetes educators. They work closely with the podiatry team to ensure fast access to diabetes podiatry clinics.
The Diabetes Psychological Wellbeing Service provides psychological assessment and goal-orientated interventions to patients who are struggling to cope with or manage their diabetes.
Who is this service for? Pre-diabetic and diabetic adults at every stage of their diabetes.
How to access this service: Referrals usually come from care providers, GPs, practice nurses or hospital consultants.
Diabetes and High Risk Foot Service: NHFT offers a Diabetes and High Risk Foot Service at 13 locations around the county. The service is for Patients with diabetes and other high risk foot conditions who are vulnerable. It's also for children and young people under 18 years of age. Diabetic Eye Screening Programme: NHFT offers a Diabetic Eye Screening Programme at 9 locations around the county. The screenings are to detect diabetic retinopathy, and they take place at least once a year. The programme is open to all diabetic patients registered with GPs aged 12+.
