Description
West Northamptonshire Council's Reablement Therapy Service is for adults who want to optimise their functional independence in completing daily activities.
The service will enable you to live your best life with your disability or long-term condition. It aims to reduce barriers and connect you to your local community.
The service provides reablement. This means it teaches people to learn or re-learn the skills needed to engage in everyday activities.
The Reablement Therapy Service also provides support and advice for people who have a disability or long-term condition.
The team consists of physiotherapists, occupational therapists, and therapy assistants.
About the Assessment
The service offers an assessment to establish your needs (relating to your disability or long-term condition). It looks at the impact your needs have on your daily routines.
During your assessment, information is gathered about:
- what matters most to you
- your strengths (what you can do)
- information about your disability or condition
- the difficulties or challenges you could be facing
The assessment is a two-way open discussion and will cover all relevant areas of your life. The information gathered is used to agree a person-centred and strength-based plan.
Assessments usually take place at home but can also be at work, a day centre, or another residential establishment. They consider your physical, social, and cultural needs (and those of your carer).
Individual Reablement Programmes
Reablement programmes can enable people with a physical disability to regain confidence and develop independence.
These include areas such as mobility and daily living activities (both inside and out in the community).
