Your Day Service – Looking to the Future

We want to hear about your experience of the day service, what matters to you, and what you think we should consider when we look to the future when providing services. 

How to have your say 

It takes 15 to 20 minutes to complete. Your answers are anonymous and will help us understand what people think about the options.  

Your feedback will be carefully considered and included in a report that will be presented to a future Cabinet meeting later this year, where decisions will be made about day services. 

You do not have to answer every question, you can skip any you are unsure about. You can also ask someone to help you, but it’s important that your answers reflect your own views and experiences.
