Description
The service is tailored to prevent decline, and to support and manage the health and wellbeing needs of anyone eligible over the age of 18, who may be showing signs of reduced independence.
With up to 12 weeks of one-to-one support, the team of dedicated Wellbeing Advisors are committed to providing you with the knowledge and resources needed to make your wellbeing a priority.
To self-refer or make a referral on someone else's behalf, please complete the short eligibility assessment.
