Description

NHFT hosts two community teams for people with learning disabilities (CTPLDs) and the Intensive Support Team (IST).

The CTPLDs provide specialist healthcare service for people who have a learning disability (IQ below 70) and health need that requires input. The focus is on supporting people with learning disabilities and related health needs to lead happier, healthier, and more fulfilling lives. The teams are made up of up of skilled multi-disciplinary practitioners, clinicians, and nurses.

The IST provides community assessment and treatment to adults with learning disabilities experiencing mental ill health or behaviour that challenges. This multi-disciplinary team (of nurses, a psychologist, occupational therapists, and speech and language therapists) is supported by a duty clinician who responds to urgent and deteriorating needs.

NHFT also has a respite ward for short stays.

Who is this service for? All ages with a learning disability (IQ below 70).
