Community Children's Nursing Service (West) - NGH

The Community Children's Nursing (CCN) Service consists of a lead nurse, senior staff nurses, and children's support workers. They provide a comprehensive nursing service to children and young people (CYP) who have nursing needs that cannot be met by universal services. This includes:

• Those with acute or short-term conditions
• Those with disabilities or complex conditions
• Those with life-limiting or life-threatening illnesses (including those requiring palliative and end of life care) 

The CCNs provide care to:

• Children aged 0-18 years
• Children who are residents in south Northamptonshire
• Children who are registered with a GP in West Northamptonshire area (whose GP is part of the Nene Commissioning Group)

Children who meet the criteria would include those who:

• have palliative or end-of-life needs
• are enterally fed
• have oxygen requirements
• require suction
• have a tracheostomy
• require saturation monitoring
• require nebulised therapy
• require post-operative wound care or catheter removal
• require ongoing management of antegrade continence enema (ACE) procedure
• require long-term home ventilation
• require maintenance of central venous access devices
• have a pressure ulcer

Referrals are accepted from professionals via the Referral Management Centre (RMC).
