Description

The Children's Community Nursing Service supports children and young people aged 0 to 18 years who have ongoing health conditions or nursing needs.

The service helps children and their families manage both short-term illnesses and long-term health conditions. Support can be provided at home, in schools, in community settings, and in hospital.

The service supports children with a wide range of conditions, including:

  • Genetic conditions
  • Breathing and respiratory conditions
  • Cancer
  • Arthritis
  • Other complex health needs

The aim of the service is to help children stay well and reduce the need for hospital stays. The team works with families to build confidence and skills so they can help manage their child's health needs at home.

Teams within the service:

Children's Community Nurses - who support children with long-term health conditions.

The Care, Assessment, and Treatment of Children at Home (CATCH) Team - which helps children who are unwell receive care at home whenever possible.

The Clinical Nurse Training Team - which provides health training for staff working in early years settings and mainstream schools.

The Children's Continence Service - which supports children and families with bladder and bowel problems.

This service is for children and young people aged 0 to 18 years who have ongoing nursing needs or long-term health conditions.

How can a child be referred?

A referral can be made by a healthcare professional or education professional involved in the child's care, such as:

  • GPs
  • Paediatricians
  • Physiotherapists
  • Health Visitors
  • School Nurses
  • School and education staff

Referrals can be made through the Northamptonshire Referral Management Centre (link).
