Preventative Services: Community Therapy - WNC

West Northamptonshire Council’s Community Therapy Service aims to support adults in living their best lives with their disabilities or long-term conditions. Support can include:

• an assessment
• support
• advice
• loaning equipment from the Integrated Community Equipment Store
• arranging minor adaptations
• recommending major adaptations
• making referrals to other specialist services

The Community Therapy Service also provides assessments for children with disabilities or long-term conditions who may benefit from adaptations to the home. 

About the Assessment 

The service offers an assessment to establish your needs (relating to your disability or long-term condition). During your assessment, information is gathered about:

• what matters most to you
• your strengths (what you can do)
• information about your disability or condition
• the difficulties or challenges you could be facing

The assessment is a two-way open discussion and will cover all relevant areas of your life. The information gathered is used to agree a person-centred and strength-based plan.

Assessments usually take place at home but can also be in a centre where therapists are based. They consider your physical, social, and cultural needs (and those of your carer).
