Community therapy

The Community Therapy Service aim to support you to live your best life with your disability or long-term condition and connect you to your local community.

The service provides assessment and recommendations for adults alongside additional support and advice.

This can range from the loan of equipment from the Integrated Community Equipment Store, arranging minor adaptations, or recommending major adaptations. We may also make an onward referral to another specialist service. 

We also provide assessments for children with disabilities or long-term conditions who may benefit from adaptations to the home. 

The service will provide a free assessment, support and advice.

About the assessment 

The Community Therapy Service offers an assessment to establish your needs relating to your disability or long-term condition.

During your assessment, information is gathered about what matters most to you, your strengths (what you can do), information about your disability or condition, and the difficulties or challenges you could be having.

This is a two-way open discussion covering all areas of your life which are being affected. The information gathered is used to agree a person-centred and strength-based action plan of recommendations.

The assessment generally takes place at your home but could be in a centre where therapists are based. It takes into consideration your physical, social, environmental, and cultural needs, and those of your carer.

Benefits of an assessment include:

  • that your individual needs are identified
  • a plan is made with you to meet those identified needs
  • we provide the most up to date information in relation to equipment and technologies which may help you
  • access to equipment which can support your safety and independence such as chair raisers, bathing equipment, toilet frames, and more specialist equipment
  • access to minor adaptations to your home to meet your needs such as grab rails and stair rails
  • recommendations for major adaptations to your home to enable safe and independent access to facilities, such as stairlifts or walk in showers
  • that you will be supported and informed about other services which may help you live your best life in your own home

Parkinson’s UK - provides support to people with Parkinson’s and their families.

Alzheimer’s Society - provides support to people with Dementia and their families.

Huntington’s Disease Association - provides support to people with Huntington’s Disease.

Age Well Northamptonshire - provides support to older and frail people in Northamptonshire.

Disabled Living Foundation - a non-profit national charity providing information and advice on daily living equipment for older adults and disabled people.

Scope - a disability equality charity in England and Wales providing practical information and emotional support when it's most needed.
