Description

Who is Supported Living For?

Supported Living services are for adults aged between 18 and 65, who have eligible care and support needs as per the Care Act 2014 (Link)

What is Supported Living?

Supported living is:

  • about having your own place to live, with the support you need
  • being independent with your own front door key
  • being able to live on your own or with friends
  • choosing to rent or own your own home
  • choosing where you live and who supports you
  • being able to live an ordinary life and contribute to your community

There are two parts to supported living:

  1. Help to find the most suitable home, whether it’s on your own or sharing with other people
  2. The support you need to do the things you want to do and to live independently

The support you get builds on the things you can do, and depends on what other support you have from your friends, family and the community.

How to Get Supported Living:

A social care professional must refer you to this service.

If you've never used Adult Social Care services before, you can use this online form to help the Council understand any care needs you may have. They will use this information to decide if you might be eligible for support.

Eleanor Lodge

West Northamptonshire Council (WNC) offers long-term Specialist Support for Younger Adults for people between the ages of 18 and 65. It is delivered out in the community and within a supported living service at Eleanor Lodge in Northampton.

Supported Living Framework

In addition to services provided at Eleanor Lodge, WNC also has a Supported Living commissioning framework. A commissioning framework is a plan that helps councils decide what care people need, who will provide it, and how to make sure it’s good and fair.

WNC's supported living framework covers five types of need. Adults with eligible needs can be supported to find the right service. The needs are grouped into five “lots,” which are:

  • Lot 1 - Learning Disability (LD)
  • Lot 2 - Autism (ASD)
  • Lot 3 - Mental Health (MH)
  • Lot 4 - Physical Disability (PD)
  • Lot 5 - Acquired Brain Injury (ABI)

Supported Living Framework Providers

Being part of this framework doesn’t mean the council recommends the provider. Please check carefully to make sure the service is right for your family member.
