Long-term care at home

This service is referred to as Specialist Support for Younger Adults and is offered as a long-term service for people between the ages of 18 and 65.

This is delivered out in the community and within a supported living service at Eleanor Lodge, Camborne Close, Northampton.

What the service can do for you 

Long term care at home service supports people with complex care needs to remain at home and to live as independently as possible

The team will support you to manage your daily routine, such as:

  • personal care (washing, dressing, bathing)
  • getting dressed and undressed
  • getting into and out of bed
  • toileting and promoting continence
  • meal preparation
  • moving safely around your home
  • support with shopping
  • support during holidays
  • budgeting, finance management and tenancy support
  • medication management
  • access to education and work opportunities (paid or unpaid)
  • household duties
  • access to social activities

People who can use the service

You can use this service if you are aged between 18 and 65 and have eligible needs as per the Care Act 2014.

How you get the service

A social care professional needs to refer you to this service - find out how to contact Adult Social Care.

What happens once you are referred

When the service receives your referral, a member of home care team will contact you to arrange a visit to your place of residence to validate your care needs and to agree your care and support plan.

When this is complete, a service start date will be agreed and service will commence for people requiring care in the community.

Supported living - Eleanor Lodge

If you have been assessed as also requiring accommodation at Eleanor Lodge, the team will talk through:

  • what happens after the visit
  • how they will match your preferences to the right shared flat

This will start with a tea visit to meet others who live within the shared accommodation.

Once it is agreed that you are happy to move into the shared accommodation, the team will:

  • link with the landlord to go through the tenancy process
  • support you to complete all relevant paperwork

How the service is reviewed

The social work professional will carry out statutory reviews however the service will continue to work with you in reviewing the support offered.
