Help after you come home from hospital
You may be able to get free care and support at home for up to 6 weeks after leaving hospital or to prevent you going into hospital. This is called intermediate care or reablement.
It aims to help you become as independent as you were before. This support could include equipment or changes to your home to help you move around, or help from a paid carer with tasks like cleaning, washing, dressing and using the toilet.
If you have savings and assets worth £23,250 or more, or a high enough weekly income to pay for care home fees, you will not qualify for help with care costs. You are considered a 'self-funder'.
As a self-funder, you will need to pay for all your care costs. We have information to help you find ways to do this.
